Clearance Document Submission Form

This form is used to gather information about a new clearance document. When this form is submitted it will move through the clearance approval and signature process. Please provide all relevant information pertaining to the clearance document below.

Note: This process needs at least 5 business days to go through the complete process as long as there are no major edits needed.

Please add the appropriate name of the document related to this submission.

Provide the purpose for submitting this clearance document.

Provide the USDA email of the document submission POC. This email will receive action items throughout the process.

Please provide your Deputy Area / Office affiliated.

Select the appropriate official for final clearance signature.

Select the appropriate document type from the drop down menu below. If your document is a "National Bulletin" please select "Directive."

If you selected "Directive" to the previous question please add all Division(s) / Offices(s) that need to complete the approval / signature of the Directive.

If other organizational units are impacted by this submission outside of your Deputy Area / Office , select "Yes" from the dropdown menu. If not then select "No". Please select "No" if the document type is "FPAC-ISD Generated Documents."

If the answer to previous question is "Yes" please use the dropdown menu to provide the other Deputy Area(s) / Office(s) affected. If the answer to previous question is "No" please leave this field blank.

Please add the deadline to complete this clearance document.

Please upload the Clearance Document that will need to be signed and approved.
